Hi Catherine,
Please find attached a completed time sheet for w/c 14.07.21 plus I worked
from 09.00 to 12.30 on Monday 21.07.21. I have also attached the new
starter document.
On the new starter document provided, I have not been able to fill in the
section �Employee statement� as none of the three categories fit my
situation. I confirm this is my only job since 6th April 2021. I claimed
contributory based JSA until the 25th May 2021 and have received a P45
stating total pay �533.17. If you need a copy P45 I can send you a
screenshot if that is acceptable? I also started to receive a private
pension in May 2021 on which I have been taxed too. I am not up to date
on what is needed for payroll currently.
My Bank details are: First Direct 40-47-64 32153637.
Please let me know if your need any further information from me.
I would also like to take the opportunity of thanking you for your
understanding and support following what I can only describe as a very
challenging week due the employer�s poor attitude not just to myself. I
found him impossible to work with!
