Hi Jay,

I can send you the forms on Tuesday no problem. If you could let me 
know if you want to do it over 6, 9 or 12 months. To be honest we don't 
have much more info other than what I've already said haha. It's fairly 
simple to fill in the forms online but you can always give us a call if 
you get stuck. We don't get here till 11am on Tuesday morning so it 
might not be till the afternoon that we get them to you if that's ok?
