Dear Mr Home,

Welcome to our practice.  This letter is to confirm your appointment on
Tuesday 15/08/2017 11:35 with Dr Amy Joomun.

Please print and complete the attached medical history form and bring it
with you to your appointment.  If you are unable to get up the stairs
easily, please inform us in advance so arrangments can be made for you.

Your appointment has been reserved on the above date exclusively for you.
If you are unable to attend for any reason, please ensure you give us a
minimum of 24 hours notice.  If this initial appointment is missed, we may
be unable to see you again on an NHS basis.

If you are travelling by car, roadside metered parking is available
outside the practice.  The current parking charge is 12p every 6 minutes
(subject to change).  All our dentists try their very best to run to time,
but sometimes unavoidable delays may arise.  We apologise in advance for
any wait you may experience.  If you are running a tight schedule
yourself, you may find it helpful to telephone the practice to enquire
whether your dentist is running on time and inform our receptionist on
arrival.

Please note that we do ask for full advance payment for any treatment you
require in accordance with current NHS guidelines but you will be exempt
from charges if any of the following apply to you:

Under 18 years old                                                   18
years old and in full time education
Income Support
Had a baby in past 12 Months
Tax Credits (with Valid Exemption Card)                   Pregnant
Guaranteed Pension Credit                                       Valid
HC2 Certificate
Income based Jobseeker Allowance                         Employment and
Support Allowance

Please bring proof of your exemption with you.  If you have to pay, we
accept cash, all credit and debit cards.  Sorry, we do not accept cheques.
